Kegerator Forum: No luck with first Tap

Tapped my first 1/2 keg of Sierra Nevada on a brand new Frosty Kegerator.  Temps fine.  Checked and triple checked all connections tapped keg and have CO2 at 12 PSI.  Day 1---Beer (5-7 pitchers) comes out veryfast and 3/4 foam.  Once settled taste ok not great.  Dropped PSI to 6-8 and beer comes out slower but immediately flat.  Vented and shut all off and reattached in order.  Set PSI to 10.  Left alone and next morning PSI down to 4.  Disconnected again and found hat washer was torn.  Vented, disaasembled and reassembled with new hat washer.  Set PSI between 10 & 12 and let sit for 24 hours.  Today back to the original problem.  At 10-12 PSI have very fast pour with 3/4 foam and ok taste that dissapates quick.  Have not gone with low PSI but after a 2o PSI charge for 1/2 hour vented and set to 10 and the first were flat.  back at 10-12 and sit for hour no pressure drop and same 3/4 foam with ok tast that goes away quick.

Any help?  I wonder if I have a bad keg although it shows it was made on 6-10-11.

Also concerned that when PSI dropped to a good pour rate that beer is instanly flat and like water no head or bottom carbonation.  Not saying zero pressure but 4-6 reading.
