Kegerator Forum: Possible "Keg Shell" issue?

For the past year I am the guy people come to in the area for kegerator issues. Yesterday I went to customers house to check out his set up, he said he was having foaming issues so I thought "No problem". I expected the first pour to be mostly foam and the second to be fine. After waiting 5 min. we would pour again, and yep, FOAM. I checked all the seals and replaced washers, psi is set properly and fridge temp is 40. I noticed bubbles forming in the beer line as if they were back tracking into the keg slowly. The check ball appears to be fine so I just can't figure this one out. Customer stated he connected a slim keg up to it over the weekend and it poured just fine, but switching back to his keg of Dunkel starts the problem again.

I would also like to point out that this is a converted fridge, so it isn't a warm tower issue.
