Sounds like you might need to rebuild the regulator or get a new one.

Do this first. Turn off the CO2 and bleed the air out of the line. Then turn the set screw all the way off. (which should be out as far as it can go) Then turn the CO2 back on and slowly start to increase pressure giving it a few seconds after each small turn to settle.

See where that gets you. If it still keeps rising you probably have a bad regulator.

Also you've probably over-carbonated the beer with the 40PSI in the keg. To fix that you'll have to do a keg shakedown. Turn off the CO2 and bleed all the air out of the keg at the coupler (Tavern Head). Then pick up the keg and shake it a few times, put it back down and bleed the air that built back up in it. You'll have to do this 3-5 times to get the carbonation out of the barrel. After that if your Gauge reads 12-14psi again you should be good to go.
